Health Forms

The Physician Clearance Form and Health History Questionnaire are the most important pieces of information we obtain from parents and guardians. This information tells us everything from allergies to current medical conditions to emergency contacts and health insurance details. These provide our nurses and your child's unit staff with vital information to provide good care while they're with us.

Health History Questionnaire: You fill this out as part of Ultracamp registration. We will ask you to update it closer to camp to reflect any changes between registration and camp starting. By signing this section of registration, you also provide necessary Medical Authorization to treat your child if necessary. 

Physician Clearance Form: Every camper
 must have a physical exam by a LICENSED PHYSICIAN - MD; PHYSICIAN'S ASSISTANT or a NURSE PRACTITIONER ACTING UNDER THE SUPERVISION OF A LICENSED MD within 12 months of camp attendance. Your medical provider then needs to fill out this form and clear your camper for camp.

Our medical staff reviews all health info so it is important to have this info in before the May 15 deadline. The Health Director or a camp nurse may call you if there are any questions after reviewing your child's health information.
